1. Application form should be signed by a person in whose name the telephone is working or by the authorised person (s) in case, it is working in the name of a firm, company etc..
2. Application should be filled in Block Capital Letters.
3. Cases for phone-plus services are : (A) Call waiting (campon) (B) Abbreviated Dialling (C) Hot lines (D) Call Transfer (call forwarding)
4. Please make a xerox copy or type the form in the format received on fax, on a good quality paper preferably bond paper and fill in the application neatly.
5. Filled in application form should be submitted at Divisional Customer Service Centre in whose jurisdiction the telephone is presently working.
